Digital Grand Rounds - Case #92

History / Objective Data

(This DGR case requires a VBScript-compatible browser to work optimally. You should either be using Microsoft Internet Explorer, or Netscape equipped with the NCompass ScriptActive plug-in. The answer can be viewed via straight HTML, as well... see the link at the end of the case history.)

This case was graciously donated by Michael Gerstner, OD of the Southern College of Optometry in Memphis. My thanks go to Dr. Gerstner!

A 61 y/o black female presented with complaints of reduced vision in both eyes. She had a positive history of diabetes with current control via Insulin. She also reported hypertension and congestive heart failure. Accupril, Lanoxin, and dialysis were part of her treatment regimen.

Best corrected vision was 20/50 in both eyes. Cataracts were found and surgery was performed on the right eye. No other ocular abnormalities were found prior to the surgery, including diabetic retinopathy.

Post-op findings on day one were normal. But at 12 days, the patient returned to the clinic with ocular complaints of reduced vision, pain, redness, and photophobia. Vision had dropped. Her corneas were edematous, and she reported nausea. Her biomicroscopic findings can be seen here.

What is this condition and how do you think the patient was managed?
