The limbal changes seen here are an extreme example of Tranta's dots. This is a beyond-textbook case of limbal vernal keratoconjectivitis. Alomide has been shown to be very effective as a mast cell stabilizer
for vernal, perhps more so than chromolyn sodium. As with any mast cell stabilizer, it may take weeks before there is any beneficial effect.... so many of these patients are typically placed on a pulse
steroid regimen, tapered quickly as vernal patients tend to like steroids a lot! Pred Forte combined with Alomide would be the best treatment for this patient... QID with the steroid, then tapered.
