
Well... I thought I had the last case by Robert McClenathan. I was wrong. One more... and here it goes.
A 28 y/o white male presented in the office with complaints of significantly reduced distance vision in both eyes some months previous. He said that his vision had improved but was still ""not quite right". He denied any significant family history, or personal history of systemic disease, or eye abnormality.
Uncorrected acuity was 20/25- in both eyes. Refraction revealed a small myopic correction with 20/20-2 in both eyes with lenses.
Biomicroscopy was normal, as well EOM testing, and IOP. Pupils were normal as well. No lens opacities were noted.
Upon dilation, small lamellar macular holes were seen in both eyes. The photo to your left was of the right eye, as was virtually identical to the lesion seen in the left.
What condition(s) would you suspect? The hints will pretty much give it away, so think hard before you start hitting the hint button.
