Case #46

Diagnosis? Subluxated lens. See how easy that was? What I'd like to do here is really have a refresher on the differential diagnosis of lenticular subluxation. It's one of those conditions most ODs will see once in a while and it's important to be able to properly work up one of these patients.

Basic info on this patient:

24 y/o black male, presents with complaints of poor vision in the right eye. Uncorrected VA is 20/200, with monocular double vision. This had been going on for about a month. IOP was normal; pupils were normal; no cell or flare or other anterior segment problems. The retinal findings were normal in both eyes. VA in the left eye? 20/25.

Click through the hints on this one and you'll find the etiology to be pretty straight forward, but do make sure to read the answer because we'll discuss the differential and work-up in some detail.
