
A neat case. One that we should all keep in mind as we go through our busy days.
A 45 y/o white male presented with complaints of near vision problems (surprise!) and no other real complaints. A bit of itching at times, but other than that... nothing.
His distance vision was 20/25+ in the right eye, 20/20 in the left, with minimal refractive error. Biomicroscopy was normal, as were his IOPs. Pupils were also fine. There was no afferent pupil defect.
Upon dilation, the right eye revealed the image seen to your left. The left eye was somewhat similar, but slightly less dramatic.
What is the diagnosis, and what management most likely ensured?
