This is a case of medulated nerve fibers. The sheer size of the area of myelination is unusual, as well as the fact that these fibers do not originate at the optic nerve, which is the norm.

Medulated nerve fibers are considered a normal retinal variation and are of not significant concern. They are not progressive, but should be considered if visual field defects are ever found in such a patient. Myelination can cause field loss in the overlying area.
