Digital Grand Rounds Case #118

A 38 y/o black male presented with complaints of painful eyes and blurred vision over the past week and a half. He reported that he was HIV positive and currently on several medications including Septra, Acetazolomide, Stavudine, Viracept, and Diflucan. No other significant history was reported.

Uncorrected vision was 20/40 in the right eye and 20/50 in the left. Vision was correctable to 20/25 in both eyes.

Pupils were normal. IOP was 12 mm Hg in both eyes.

Lids showed some blepharitis, but anterior segment evaluation under slit lamp was otherwise completely normal.

Dilated fundus evaluation revealed the optic nerve findings shown here. Both nerve heads were similar in appearance.
