Digital Grand Rounds - Case #110

History / Objective Data

(This DGR case requires a VBScript-compatible browser to work optimally. You should either be using Microsoft Internet Explorer, or Netscape equipped with the NCompass ScriptActive plug-in. The answer can be viewed via straight HTML, as well... see the link at the end of the case history.)

A 27 y/o male presented with a bizarre history of tears spontaneously "gushing" from an unknown source in his left eye. This had been going on for the past six weeks. He denied rubbing his eye or any history of trauma. He also denied any vision disturbances. No other significant ocular, medical, or family history was reported.

Uncorrected acuities were 20/25 in the right eye and 20/25 minus 2 in the left. Minimal refractive error was found… just a slight amount of astigmatism, which when corrected gave solid 20/20 vision in both eyes.

EOMs were full and smooth. No restrictions were noted. Pupils were normal OU. No afferent pupil defect was found.

IOP was 19 mmHg in the right eye and 20 mmHg in the left.

Dilated fundus examination was normal OD and OS. Anterior segment examination with biomicroscope was normal as well, with the exception of the large "cyst" seen in this photo when the upper lid was retracted.

What is this condition and how should it be managed?
