A 42 y/o black male presented with complaints of decreasing vision in the right eye. He said he had noticed a spot in his eyes as well as great difficulty driving at night.

His habitual correction gave him vision of 20/25 and 20/30 in the right and left eyes respectively. Best correction gave no improvement.

His EOMs and pupil responses were normal. IOP was 15mmHg OU.

Biomicroscopy showed normal corneas and anterior chamber findings, but early nuclear changes in the lenses of both eyes.

Dilated fundus examination showed the findings depicted in the photo here for the right eye.
